Implant-Prosthetic Rehabilitation of Severe Maxillary Atrophies with Custom-Made Subperiosteal Implants

The implant-prosthetic rehabilitation of severe maxillary and mandibular atrophies represents a significant clinical challenge. These cases often require multiple surgical procedures and long rehabilitation periods, with long-term outcomes that are frequently unpredictable.
Juxta-osseous implantology was first introduced in the 1940s and, despite alternating success over the decades, has persisted into modern practice. These earlier techniques were often associated with high long-term failure rates, mainly due to the difficulties in achieving precise implant-to-bone fitting and the lack of application of modern rigid osteosynthesis principles.
Recently, advancements in 3D prototyping have helped to overcome many of these limitations, making it possible to manufacture custom-made implants with precise anatomical fitting. This allows for single-stage surgical treatment and immediate functional loading in selected cases.
